To obtain services through CCP, you must first select a home health agency for your child. Once you have decided on an agency, an RN or Therapist from that agency will come to your home to evaluate your child for services. The agency RN or Therapist will coordinate with your child’s physician to determine the amount of hours for PDN or visits for Therapy your child may qualify for. The agency will request authorization from the state through Texas Medicaid and Healthcare Partnership (TMHP) and should receive an authorization within a week’s time. An authorization for PDN comes in hours per week. You cannot exceed these hours but if you go under you lose the hours not utilized. Authorizations for this program are anywhere from 90 to 180 days long. An authorization for therapy comes in visits per authorization period which is also from 90-180 days long. This gives you the flexibility to have therapy services twice per week, for example, but if your child is sick and must miss a visit, you can make it up the next week. Any visits not utilized by the end of the auth are lost.
